It is new in the box and comes as shown in pictures. Per manufacturer: " This Bracket Kit is Required for Installation of a Standard 5th Wheel on Dodge RAM trucks with OEM factory puck system. This unit is easy to install and easy to remove. The frame rails easily install in the top side of the truck box with all adjustments being made from in the truck box without having to make any adjustments from the underside of the box. Four Locking pins secure this unit to the OEM factory underbed brackets. Rail system can be removed with or without the fifth hitch attached.
